On an ethical standpoint, what becomes of people with severe mental handicaps?

I mean the people with handicaps that render them dysfunctional in society. I've always been a supporter of Darwinism but recently i think as caring human beings we've taken care of them well, but what ultimately becomes of them? Are we going against natural selection by helping them?
